How do I apply for a U.S passport for my child?

You need to submit your child's passport application in person to a passport acceptance facility. Passport acceptance facilities include post offices, courts, and some libraries. Your local post office is usually a great place to start. Note that not all post offices offer this service, so it is a good idea to either call your local post office or check online before you head out.

If you need a passport urgently (within 2 weeks), you can contact a Passport Agency that can provide expedited service for an additional fee.

Some things to note:

  • In a two-parent home, both parents are expected to be present to submit the application. If one parent can not be there, then that parent needs to fill out a separate form (Form DS-3053) that gives consent for the passport to be issued.
  • Also, there is a new requirement stating that the child also needs to be present at the time of the application for a passport.

Also, before you start, make photocopies of all your adoption-related documents. You are supposed to submit originals with the passport application, but sometimes this varies depending on the clerk. If you bring both the originals and copies with you, then you will be prepared to submit whichever one the clerk wants. If you do end up needing to send in the originals, then you know that you have a backup copy just in case.

What will I need tp apply for a passport?

You will need an "Application for a U.S. Passport or Registration (form DS-11) downloadable from the State Department website or available at your nearest passport accepance facility (e.g post office).
